Days No Pay/Same As CASH.*
To apply for a JC Mattress Credit Card,
follow these simple steps:

request
an application, which will be emailed to you.
- Please complete
and sign the application.
- Mail the completed application form
to the
address listed below:

- Once we receive
your completed and signed application, we will
process the application and a representative will call
you to discuss
the loan
decision.

via phone; however, a completed application must be mailed
to us for
compliance.

your JC Mattress credit card, subject to credit approval.
Finance Charges
will accrue on the purchase from the date of purchase
but no minimum monthly payments will be due prior to the promotion
expiration date as indicated on your billing statement, and if
you
pay
the
cash
sales
price of the purchase by the promotion expiration date,
no Finance
Charges will be due on the purchase. Annual Fees: None.

or Beneficial.
